Need Help with Secondary Quest (Mythril Stanza)

Hi Guys, I'm new here and I really wish to get some help.
I'm sorry if I post in the wrong place

So, I haven't played DSII for so long and today I decided to complete all the chants, bestiary, etc.
But I'm stuck with one thing. I couldn't get the Mythril Stanza...I did get the other Stanzas, but when I tried
to open the chest of Mythril Stanza, It was already opened but the item is not unlocked in my journal, therefore I cant finish
the quest which I couldnt get the chant =(

Anyone can help me? what am I missing? I've tried many things and Im starting to give up.

Thank you so much for replying to this thread...I have seen all the links that u've given to me but I think it still didnt help =(

I attached you the screenshots so maybe you can understand better what my problem is...
the first picture is where the mythril stanza is suppose to be (but the treasure is opened)
the second picture shows that the item isn't shown in the journal

Again, thank you so much for the help, btw I am not using any mode.

i recall this is a bug with earlier unpatched versions of ds2. are u running ds2 v2.2? if so, patch your game and then revert to an earlier backup copy of your save from before u entered the mines of kaderak. it *might* fix it.

if the game was started from before u patched and patching still doesnt fix it, u have to restart from scratch in eirulan.

if your game is already patched to v2.2, sometimes ds2 is just plain unreliable and prone to random glitches. if u encounter glitches like this, starting over is the only way. i personally havent run into any such game-stopping random glitch issues like this before myself.
